Ad Fontes Academy student-athletes must have on file, in the school office, the following completed forms: ˑ

1. 1. An official VHSL Athletic Participation/Parental Consent/Physical Examination form.
2. 2. A signed Ad Fontes Academy Athletic Participation Agreement
3. 3. A registration form for each sport.
